Notification of Student Transfer Form

Students transfer between schools for many reasons, and compulsory attendance laws require public school administrators to verify the transfer is legitimate or authorized. PIE’s Notification of Student Transfer form placates this necessity by notifying your child’s current school of your intentions to relocate your child to a home school setting under the auspices of Palmetto Independent Educators Association.

Complete the following form by selecting the submit button at the end of the questionnaire, then the return button, repeating the process for each student. Current members may enter their membership number (e.g., 0858M96, 0858M, 0858).

  • ATTENTION: Signature Required

    The office of PIE will send the Transfer Notification Form to your current school through fax or email. This form is processed through the office of PIE because it needs to be signed by the director before completion.

  • The office of PIE will request Educational Records be mailed, faxed, or emailed to the office of PIE, which will include:

    a: Transcript of high school courses completed, and credits earned to date
    b: Courses in progress and numerical grades to date of withdrawal
    c: Key to your grading system
