  • All Members: Complete a Transfer Form for all students transferring from a public or private school.
  • New Members: Upload a copy of your parent-teachers diploma or GED certificate for parents and guardians with your online application, or click to upload here, or email a copy of the parent-teachers diploma or GED certificate for parents and guardians.
  • New Members: Upload a copy of your court-ordered guardianship documentation with your online application, or click to upload here, or email a copy of your court-ordered guardianship documentation for guardians.
  • Existing Members: complete your previous year "School Year Review Form" per student.
